That's why we ask you to complete our new client and patient history forms and, when applicable, a seizure questionnaire before your pet's initial appointment. Click on the appropriate forms and either complete and submit them to us online or download and fax them to us at 415-380-0504. You can also bring the completed forms to your pet's appointment.
