Referral information

Cases may be referred at any time. No referral form or call is necessary.
All cases will be sent back to the referring veterinarian. Radiograph review is done at no charge.

Please email or fax pertinent labwork prior to appointment or alternatively have owner bring along on appointment/surgery day.

For digital radiographs, please send disk or have owner bring disk on appointment/surgery day. Please do not email digital xrays. (This often does not allow a high enough resolution)

Veterinarians wishing to contact the clinic, Dr. Sams, Dr. Klopp or Dr. Hawley should call the Front Desk. Email available upon request.

How to send a patient for a CT scan

No consultation is necessary.
In order to provide your patient with the best care and to get the best clinical interpretation of the scan, please ensure the following points are covered:

1. Fill out the Referral Form (available by fax from our office or by download
above). On this form you tell us which part of the body is to be scanned and
whether or not to use contrast.

2. All pertinent x-rays should be brought with the patient on the day of the scan
These are sent with the CT scan to the Radiologist and also help us take
the best scan possible.

3. A complete history is needed for the Radiologist to help interpret the CT
scan. We overnight the results to your Radiologist using Fedex.

3. Bloodwork results (CBC, chem) dated within two weeks prior to scan

4. Your anesthesia evaluation including any concerns.
(Included on Referral Form) Please call Dr. Hawley (Anesthesia Specialist)
to discuss any concerns you may have.

We look forward to providing you with state-of-the-art surgery, treatments and diagnostic imaging services for you and your patients.
